Please Complete the below forms prior to your 1st Session with Dr. Crider and bring to your first session. The Release of Info Form does not need to be completed unless requested. The Colorado Notice form is read only.

Please let Dr. Crider know if you would like to fill out the paperwork upon arrival. You will need to come 15 minutes prior to your scheduled appointment. Take a seat in one of the chairs at the top of the 2nd floor landing and you will find the paperwork on the table next to you.
